Consent and authorization forms are common for family, school, travel, medical, business, and administrative needs. I can perform the notarial act when the document is complete, the signer personally appears, and an acceptable method of identification is available.

Who decides which people must sign?
The organization receiving the form, the document preparer, or an attorney must answer that question. I can notarize an eligible signature and complete the certificate, but I cannot decide whether one parent, both parents, a guardian, an owner, or another authorized person must sign.
Before requesting the appointment
Ask the receiving party for the correct prepared form and confirm any witness, original-document, or delivery requirement. Send me the form type, signer count, location, and deadline so I can confirm timing and the identification method available before traveling.
